Who is eligible for Continuance Coverage?

Reliant employees covered on Reliant's domestic vision insurance plan (through EyeMed) are eligible for continuance coverage under the following circumstance:

Dependents of Reliant employees covered on Reliant's Domestic vision insurance plan (through Lincoln) are eligible for continuance coverage on Reliant's vision plans under the following circumstances:

 

Can you change your coverage when transferring to Continuance?

No, when switching to continuance coverage, there is NOT an option to change your selected vision plan and covered individuals (adding and dropping dependents etc).  The coverage offered is the same coverage that the employee had prior to the loss in benefits eligibility. 

The only exceptions that allow a change in the vision coverage are in the event of the death or divorce from the Reliant employee, if the Reliant employee became eligible for Medicare, or in the event a dependent reaches the maximum age limit under the plan (age 26) or marries, therefore a change to the covered persons on the policy would be appropriate.

What is the Cost of the Continuance Coverage?

The cost of the continuance coverage is the premium cost for the selected vision plan. There are no additional fees charged by Reliant. 

There is no longer the ability to receive the employer HSA contribution when switching to Continuance coverage.  Therefore there is no additional charge for the employer HSA contribution that was previously charged partially to the MTD account and partially subsidized by Reliant.. 

 

How long does someone have to make a decision about Continuance Coverage?

Reliant needs to receive the check for the 1st month of continuance coverage as well as the continuance coverage enrollment paperwork no later than 45 days after the date in which the Reliant employee lost benefits eligibility with Reliant.  In most cases it is preferable to make a decision about coverage well before that date, in order to provide seamless vision coverage for the employee and if applicable, the employee's dependents.

 

When are Continuance Coverage premiums due?

The initial check for continuance coverage must be received within 45 days of when the employee's benefits eligibility with Reliant ended.  The ongoing checks for Continuance coverage must be received by Reliant by the 20th of the month for which the coverage is being paid.  In most cases (unless previous arrangements have been made) if the check for continuance coverage is not received by the last business day of the month for which coverage was provided, Reliant will cancel the coverage.  

 

Can I cancel Continuance Coverage at any time?

Yes, you can cancel continuance coverage at any time by emailing us at hr@reliant.org informing us of the last day you (or any applicable dependents) to be covered.

 

Can I change my Continuance Coverage once I've selected it?

No, there is NOT an option to change your selected Domestic coverage vision plan or the covered individuals (adding and dropping dependents etc) at any time (not even during Open Enrollment).  The coverage offered is the same coverage that the employee had prior to the loss in benefits eligibility.
