Reliant employees who wish to participate in the incentive program for MTD coaching are responsible to report their coaching hours. 

Submitting your bonus request

• Make sure you are logged into your Reliant Google Account on this browser. You can do this at mail.reliant.org or drive.reliant.org.
• When you click the form link below, you will be asked to make a copy of this document. Click the Make A Copy button.

• The bonus request will now be in your Reliant Google Drive. Go to the upper-left corner of the screen to change the file name to include your name and the month and year for the request. For example, Jill Missionary - July 2017 MTD Coaching Bonus Request.

• Share this document with your Program Team (like collegiate.program@reliant.org) by using the blue share button in the upper-right corner.
