Vision Insurance through EyeMed is optionall.  The monthly insurance premiums will be deducted from missionary paychecks.

Consider joining the voluntary Vision Inusrance program.  This insurance is available to ALL U.S. missionaries, regardless of hours worked.  So you can be either a full-time or part-time staff and elect EyeMed Vision Insurance.  The premiums are paid by the missionary pre-tax through the Reliant Cafeteria Plan.  

Complete the EyeMed Vision Enrollment Form. 

Vision enrollment as well as Continued Vision Enrollment also requires submission of the Reliant Cafeteria Plan Enrollment Form. 

We partnered with EyeMed to offer vision insurance that will provide:

• $10 eye exam at in-network providers
• Annual lens and bi-annual frames for glasses after a $25 copay
• Contact lens coverage up to $120/year after a $25 copay
