This year our 1095-C forms will be available online. You can view your 1095-C at http://reliantm.greenemployee.com

Please click the link and enter the required information to log in and view your form. (You may need to create an account first if you did not already create a login when accessing your 2022 W2.)  The login and password are different than your other Reliant logins and are unique to Greenshades Online. 

When you login to Greenshades Online, you will be asked to consent to receive an electronic 1095-C in place of a paper 1095-C, and a verification code will be generated that you will need to type in.
