Reliant employees who wish to participate in the incentive program for MTD Cohort Facilitator (Reliant will initiate a transfer request to cover the amount bonused).
Submitting your bonus request
- Make sure you are logged into your Reliant Google Account on this browser. You can do this at mail.reliant.org or drive.reliant.org.
- When you click the form link below, you will be asked to make a copy of this document. Click the Make A Copy button.
- The bonus request will now be in your Reliant Google Drive. Go to the upper-left corner of the screen to change the file name to include your name and the month and year for the request. For example, Coaching Bonus Request Form - Jill Missionary April 2022
- Once you've filled out the needed information, be sure to share it with the MTD Health Team or approval.
- Once this document is approved, the MTD Health Team will send it to payroll for processing. If this is done before the 16th of the month, it will be paid on the next month's paycheck. If it is submitted after the 16th, it will be paid in the paycheck 2 months from submission.
