Section I. MANAGEMENT COMMITMENT AND INVOLVEMENT 

POLICY STATEMENT 

The management of Reliant is committed to providing staff with a safe and healthy workplace. Reliant's policy is that staff members report unsafe conditions and do not perform work tasks if the work is considered unsafe. Staff members must report all accidents, injuries, and unsafe conditions to their supervisors and the Reliant Central Team. No such report will result in retaliation, penalty, or other disincentives. Our management team will give staff members recommendations to improve safety and health conditions thorough consideration. Management will give top priority to and provide the financial resources for the correction of unsafe conditions. Similarly, management will take disciplinary action against a staff member who willfully or repeatedly violates workplace safety rules. This action may include verbal or written reprimands and may ultimately result in termination of employment.

Senior management will be actively involved with staff members in establishing and maintaining an effective safety program. In addition, the safety program coordinator, executive director, or other members of the management team will participate with local church and ministry partners’ representatives in ongoing safety and health program activities, which include:

  • Providing safety and health education and training; and 
  • Reviewing and updating workplace safety rules where applicable. 

This policy statement serves to express management's commitment to and involvement in providing staff members a safe and healthful workplace. This workplace safety program will be incorporated as the standard of practice for this organization. Compliance with the safety rules will be required of all employees as a condition of employment. 

Section II. SAFETY AND HEALTH TRAINING 

Each employee has access to this Safety Guide, through the Field Manual on Solomon, for review and future reference. Reliant will instruct all employees that compliance with the safety rules described in the workplace Safety Guide is required. 

Job-Specific Training

  • As needs arise in the field that would create potential work hazards, the employee or their supervisor will be responsible for contacting the Reliant office to inform and request appropriate training materials. 
  • Where applicable, supervisors will observe employees performing the work. If necessary, the supervisor will demonstrate using safe work practices or remedial instruction to correct training deficiencies before an employee is permitted to do the work without supervision. 
  • All employees will receive safe operating instructions on seldom-used or new equipment before using the equipment. 
  • Supervisors will review safe work practices with employees before permitting the performance of new, non-routine, or specialized procedures. 

Periodic Retraining of Employees 

Individual employees will be retrained after the occurrence of a work-related injury caused by an unsafe act or work practice or when a supervisor observes employees displaying unsafe acts, practices, or behaviors. 

Section III. MEDICAL TREATMENT AND FIRST AID 

Emergency Medical Treatment 

If you sustain a severe injury requiring emergency treatment: 

  • Call for help and seek assistance from a co-worker. 
  • Call 911 or a local emergency number and seek medical care.
  • Provide details to your Program Liaison and your supervisor for the completion of the incident report. 

Non-Emergency Medical Treatment 

For non-emergency work-related injuries or illnesses requiring professional medical assistance, you must notify Employment Services before seeking non-emergent medical treatment. If you sustain an injury requiring treatment other than emergency medical treatment or minor first aid: 

  • Inform your supervisor.
  • Reach out through your Program Team Liaison, who will connect you with an available member of the Employment Services team, or contact Employment Services at benefits@reliant.org.
  • Seek required medical assistance.
  • Provide details to your Program Liaison and your supervisor for the completion of the incident report.

Minor First Aid 

First-aid kits should be stored in an easily accessible place. If you sustain an injury or are involved in an accident requiring minor first-aid treatment: 

  • Inform your supervisor. 
  • Administer first aid treatment to the injury or wound. 
  • If a first aid kit is used, indicate usage on the accident investigation report. 
  • Access to a first aid kit is not intended to be a substitute for medical attention. 
  • Provide details to your Program Liaison and your supervisor for the completion of the incident report. 

Section IV. ACCIDENT INVESTIGATION 

Accident Investigation Procedures 

Both incidents (events that caused injury to a person) and near misses (events that could have caused injury to a person) that involve Reliant staff members (employees or associates) must be reported to Reliant Central Team.

The supervisor or leader at the location where the accident occurred will work with a member of the Central Team to perform an accident investigation.

The Central Team is responsible for seeing that the Incident/Near Miss Reports are being filled out completely and that the recommendations are being addressed. Investigation of all accidents, injuries, and occupational diseases will use the following investigation procedures: 

  • Implement temporary control measures to prevent any further injuries to employees. 
  • Review the equipment, operations, and processes to gain an understanding of the accident situation. 
  • Identify and interview each witness and any other person who might provide clues to the accident's causes. 
  • Investigate causal conditions and unsafe acts; make conclusions based on existing facts. 
  • Complete the Incident/Near Miss Report. 
  • Provide recommendations for corrective actions. 
  • Indicate the need for additional or remedial safety training. 

Notifications of incidents or near misses must be as soon as possible (preferably within one business day after the incident or near miss) by the supervisor of the staff member involved in the incident or another leader in the church or ministry. If a supervisor or leader is not available, the staff member may make the notification. 

Notification Procedure

If a staff member (employee or associate) is involved in an accident or near-miss:

  • Contact your Program Liaison as soon as possible.
    • If an employee is injured while performing ministry responsibilities on the weekend or after normal business hours, email benefits@reliant.org to notify the Employee Services team.
  • The Program Liaison and the Employment Services team will coordinate to ensure that Reliant Central has all the information needed for the Incident/Near Miss Report and, if applicable, Worker Compensation claims.
    • Keep in mind that this may require a series of emails or phone calls to the staff member and the leader/supervisor.

Section V. RECORD-KEEPING PROCEDURES

The safety coordinator will control and maintain all employee accident and injury records. Records are maintained for a minimum of three (3) years.

Section VI. DRIVING SAFETY

3. Driver Guidelines and Reporting Requirements

  1. Any employee or volunteer with driving duties whose driver’s license is revoked or suspended shall immediately notify his or her supervisor by the next business day and immediately discontinue driving duties on behalf of Reliant. Failure to do so may result in disciplinary action, including termination of employment or volunteer status.
  2. All collisions that occur in the course of an employee’s or volunteer’s exercise of his or her driving duties must be reported to the police and to the employee’s or volunteer’s supervisor. Collisions are to be reported immediately (from the scene, during the same day, or as soon as practicable if immediate or same-day reporting is not possible). Collisions involving personal injury to the employee or volunteer when exercising driving duties must be reported to Employment Services for Worker’s Compensation purposes. Failing to stop after a collision (“hit and run”) and/or failure to report collision as required by this policy may result in disciplinary action, up to and including termination of employment or volunteer status.
  3. Drivers must report to their supervisor all moving violations received while exercising driving duties within 72 hours of receipt.

4. Driver Criteria and Administration

Employees and volunteers exercising driving duties must hold a valid and current Driver’s license with current auto insurance and minimum liability limits as set by Reliant.

Employees are expected to drive in a safe and responsible manner and to maintain a good driving record. Reliant will periodically review records, including collisions, moving violations, etc., to determine if an employee’s or volunteer’s driving record indicates a pattern of unsafe or irresponsible driving and to make a recommendation to Program Leads for suspension or revocation of driving duties.

5. Driver Safety Rules

  1. Exercising driving duties on behalf of Reliant while under the influence of intoxicants and/or illegal drugs is forbidden and is sufficient cause for discipline, up to and including termination of employment. For purposes of this policy, “illegal drug” is defined as any controlled substance listed in schedules I through V of the federal Controlled Substances Act (21 U.S.C. § 812), medication or other chemical substance that (1) is not legally obtainable; or (2) is legally obtainable, but is not legally obtained, is not being used legally or is not being used for the purpose(s) for which it was prescribed or intended by the manufacturer. Thus, “illegal drugs” may include even over-the-counter medications if they are not being used for the purpose(s) for which they were intended by the manufacturer.
  2. Employees and volunteers exercising driving duties must avoid using a cell phone while operating a vehicle. All cell phone calls must occur while the vehicle is parked and/or use the phone in a “hands-free” mode via a headset or speaker. Any other cell phone use (e.g., texting, reading emails) must be done while the vehicle is parked. In addition to this policy, any applicable state or local laws regarding cell phone usage must also be obeyed.
  3. No employee or volunteer exercising driving duties shall operate a vehicle on behalf of Reliant when his/her ability to do so safely has been impaired by illness, fatigue, or injury.
  4. All drivers and passengers operating or riding in a vehicle must wear seat belts.
  5. All state and local laws for operating a vehicle must be obeyed.

6. Accident Procedures

If a Reliant employee or volunteer is involved in a collision while exercising driving duties, he or she must:

  1. Call for medical aid if necessary.
  2. Call the local police and request that a police report is made concerning the collision.
  3. Contact personal insurance company immediately, as well as the rental car agency (if applicable).
  4. Obtain the insurance information and license plate number of other parties and vehicles involved in the collision.
  5. If safe, take pictures of all vehicles involved, damage to vehicles and/or property, skid marks, collision site, location/condition of traffic signs or signals, and (if feasible and consented to) any visible bodily injuries to employee/volunteer, his/her passengers, and other parties.
  6. Contact Reliant via phone (407-671-9700) or email (HR@reliant.org) as soon as possible to inform Reliant of the collision.
  7. Send copies of his/her personal insurance policy and rental car agreement (if applicable) to Reliant.
    * This will help Reliant determine what coverages were purchased on the rental car and what personal coverages the employee or volunteer holds.
  8. Not discuss the collision with anyone other than the police, the rental car agency, Reliant office personnel, personal insurance company representative, or Reliant corporate insurance agent.
  9. Not admit fault.

Vehicle Accident Scene Photography

NOTE: Before you use the camera, be sure the accident scene is safe. Do not put yourself into a dangerous situation (vehicle traffic path, leaking fuel or cargo, etc.). 

Why photograph vehicle accidents and incidents? 

The use of a camera following any incident resulting in actual or potential damage to vehicles, property, or persons helps to ensure that the facts are properly and accurately documented.

Pictures of incident scenes tell a powerful and often valuable story of what actually happened. Overall, photos can serve to:

  • Clarify vehicle positions and probable point of impact. 
  • Confirm the presence or absence of traffic control signs. 
  • Facilitate and expedite accident investigations. 
  • Record the extent of damages to all involved units (and/or buildings, signs, posts, etc.).

Why not wait for the police or claims professionals to take pictures? 

As a driver, you are Reliant’s representative at the scene, and you can capture the information immediately before the vehicles are moved. Your action of taking pictures may also have an indirect benefit by sending a message to the other parties involved that Reliant takes accidents very seriously, and the facts are clearly recorded – less room for “my word against your word” arguments. Additionally:

  • Vehicles will be moved or altered after an accident. 
  • You are filling out an accident report anyway (pictures confirm their written record).
  • Photography can discourage (or possibly uncover) fraud. 

What should be photographed? 

  • The overall area (from all angles) of the accident/incident scene. Start outside and work in. Include the final resting place of the vehicles in relation to the rest of the accident scene. 
  • If an intersection, all four (4) directions approaching that intersection. 
  • Basic four (4) sides of each vehicle. 
  • Close up of damage, or lack thereof, to each vehicle. Photos showing no damage are just as important as photos showing damage. Also include any damage to buildings, signs, etc. 
  • Skid marks, including a point of reference (such as a pen) for measurement. 
  • Any contributing factors (construction, road conditions, etc.) 
  • Any evidence that could be moved later. 
  • Temporary road or construction signs, traffic signals, etc. 
  • Any visible obstructions like trees and shrubs, signs, other vehicles, etc. 
  • The license plate number of the other vehicles involved.

Do your best to photograph any other issues that seem pertinent at the time. 

What should I do after I take the pictures? 

  • Be sure to immediately report the incident to Reliant via phone (407-671-9700) or email notifyHR@reliant.org
  • Label the pictures with the date of the incident, location, and your name. 
  • Reliant will then forward them directly to our insurance carrier.

  1. Barb Seckler Here is the draft of the updated Safety Guide! It is in a temporary place. You'll see that it's much, much shorter. Would you (or someone from your team) review and give me your feedback, especially from a Worker's Comp perspective (is the report asking the right questions, should I add more, edit, etc.)? You can fill out as many test Incident/Near-Miss reports as you want. Right now, I own the Google Form, but eventually I'll transfer it to the notifyHR@reliant.org account and can delete all the tests. Once this is up and running, any reports that come in that indicate an employee injury would be forwarded to your team to follow up for Work Comp purposes. 

    Also, the Driver Safety policy is still in draft as of the time of this comment and I need to make updates, so don't worry about that link for right now.

Julie Thomas I'd want Tonya to take a look at this too but it does seem to give us the info we need, but, probably we'd also want to confirm the best contact info for the person - phone, email, etc so we (ES) can set-up a time to confirm the injury information and make the first report of injury to the work comp carrier.  We may also need to collect the contact info for the witnesses, unless you want us to do that separately when we do the full first report of injury.  

    1. Barb Seckler - that definitely makes sense. I'll update the report to collect contact info for both individual and witness. It's probably most efficient to just capture it all at once.

    1. Julie Thomasfor the google form - you added a field "Reliant Fund Number" but I think in the case that a supervisor has a Reliant FD# and the employee has a Reliant FD# we may want to clarify which we're asking for - maybe we could word that field "affected staff member's 4 digit Reliant FD#"
