To view the answers to these form questions you need to go back to the main page that lists all of their uploadable forms and the view application button. Select VIEW FILE for both the parental notification form and the spiritual reference form to read those answers. (The answers are actually put together on the same page, so when you click on one View File, you will see the answers for both the parental advice and spiritual references when you select either of the View Files).

Parental Concerns
If the participant answered their financiial independence questions a certain way (SEE Parental Nofication for more details). Then you will not have a parental notification form to view. It will just say "Financially Independent Over 21- Parent Reference not needed".
