Use this document to notify Reliant of any changes to a missionary's employment information.  Please select all applicable changes.  Changes in supervisor, coach or staff assignment require approval from both sending and receiving supervisors.  *Please note that some changes require additional documents/steps. 

 

Most changes require Supervisor review and approval unless otherwise noted.  Those in initial MTD also require the coach's review/approval if this is someone other than the supervisor.  

If the missionary is completing the document: Forward this notification to your supervisor and/or coach (if in initial) and request their approval.  Please include the following in your email signature block: Full Name, Date, Email, Phone #, and Fund #. 

For Supervisor or Coach: Please forward the notification to hr@reliant.org and confirm in the email that you have reviewed the change as well as why you are or are not approving the change.  For your signature please include: Full Name, Date, Email, Phone # and FD # (if you are a Reliant employee).

For changes in employment hours:

  • Changes in hours requires a new support goal calculator.
  • Changes to 30 or more hours will require enrollment in Reliant's medical insurance plan (unless a spouse or parent can carry you on a qualifying group plan) as well as Reliant's Life and Long-Term Disability Insurances. 
  • A reduction in hours below 300 will result in loss of eligibility for medical, dental, and life insurances. 
  • A change in hours will also result in a change in benefit levels related to PTO, education reimbursement, Ministry Expense Bonuses and others. 
  • Significant changes in hours (especially a decrease of more than 5 hours) will require donor notification through a prayer letter. 
  • A significant increase in hours may also require confirmation of an MTD Action Plan with your supervisor and coach. 

For changes in missionary level: 

  • A new level determination document must be completed.
  • An updated job description must be submitted.

 

For changes in ministry assignment: 

  • A new level Online Profile should be completed
  • If you have moved as a result of the change, a new Personal Information Change Notification should be completed.
