This document must be filled out to notify Reliant of any changes to your employment information. Please select all applicable changes. Changes in supervisor, coach or staff assignment require approval from both sending and receiving supervisors.

Steps to take

1. Fill out Missionary Employment Change Notification

2. Email form to supervisor

Details: Ask for your supervisor to approve the form and have the supervisor send it to HR@Reliant.orgIf you are currently in initial MTD, you will need to have your supervisor and MTD coach review and approve the form.

Some changes will require additional documents/steps.

Other possible steps

For changes in employment hours

  • Changes in hours requires a new Support Goal, which you can submit on Staffnet.
  • Changes to 30 or more hours will require enrollment in Reliant's medical insurance plan (unless you meet the waiver criteria) as well as Reliant's Life and Long-Term Disability Insurances. 
  • A reduction in hours below 30 will result in loss of eligibility for medical, dental, and life insurances. 
  • A change in hours will also result in a change in benefit levels related to PTO, education reimbursement, Ministry Expense Bonuses and others. 
  • Significant changes in hours (especially a decrease of more than 5 hours) will require donor notification through a prayer letter. 

For changes in missionary level

  • A new level determination document must be completed.
  • An updated job description must be submitted.

For changes in ministry assignment

  • Update your profile on Reliant.org 
  • If you have moved as a result of the change, a new Personal Information Change Notification should be completed.
