• First priority in any medical situation is to treat the participant or staffmissionary.
  
  
• Call Parent/Guardian. Emergency contact info can be found on their medical disclosure form on mygcm under events.reliant.org under your event and the participant’s name.

• Have both medical forms available in case needed. Each participant's medical disclosure forms and medical permission and liability waiver and release forms are located on mygcm under events.reliant.org under your event and the participant’s name. If you do not have access to internet to find their medical form, call Sarah Swann 540-998-7741.

Please inform the Reliant Summer Programs Coordinator regarding ANY incidents (big and small) with participants, interns, staff missionary or parents.  At Reliant, we receive the phone calls from parents/guardians when incidents arise so please keep us in the loop so we know how to answer their questions.

Please call the Reliant summer program coordinator first to inform them about any major incidents.  After the crisis has calmed down please document all details in an email to the Reliant summer program coordinator.  Include in the email details about the incident: dates, all participants and staff missionary involved, who was called, etc.  Continue to follow up with any changes or new developments to the incident as they occur. Please be detailed with what your write as follow-up phone interviews with all parties involved may also be necessary.  The Reliant summer programs coordinator will create an incident report to have on file at Reliant and will continue to add any additional follow-up information to the report.
