Reliant purchases a short-term supplemental insurance coverage that supplements, but does not replace their personal medical insurance. We will provide the summer program director with all the insurance cards and documents that they would need if something were to happen. The summer program director will be responsible for sharing the information of what is and what is not covered by the supplemental insurance to each participant. For additional information, review the STM GCM medical permission and liability waiver and release form that the International Liability Waiver Form and Medical Disclosure Form that each participant has to sign in order to participate in the program.
