To change your legal name on file with Reliant for Payroll and Tax purposes, send a copy/photo of your NEW social security card with your new last name to payroll@reliant.org.please submit the New Social Security Card Form linked here with a picture of your new Social Security Card that shows your new last name. This form utilizes DocuSign which is a secured platform. Once you submit your new Social Security Card, Reliant can update your name in its systems. For questions, please email payroll@reliant.org |
