Each program applicant will initially need to create a GCMID. This ID will allow them to complete their application, track the status of their application, view event fees and payments, upload required forms and make online payments towards their fees. Click here to view a Sample Online Application.
Finished participant applications will include:
- Essay questions on application Agreement Page Signatures on application
- Registration Payment
- Medical Disclosure Form completed and uploaded. (BLANK MED FORM)
- There is a separate medical form for International and Short Term Mission trips.
- Liability Waiver Form completed and uploaded (BLANK LIAB FORM)
- Financial Integrity Form completed and uploaded (if your program’s support checks are being written directly to students) (BLANK FINANCIAL FORM)
- Staff Member/Spiritual Leader Reference Form Received Sample Staff Member/Spiritual Leader Form
- Parent/Guardian Consent Form Received Sample Parent Pack
- Other forms required for your individual program uploaded
Parent/Guardian Consent
- If the participant is under 18 a parental consent form is required.
- If the student is over 18, but under 21 and still financially dependent on their parents/guardians the parental consent form and packet will be sent to their parents/guardians.
- If the participant is over 21 they do not need a parental consent form an no email will be sent to the parents. You will see it say this on their application: PARENTAL CONSENT FORM NOT NEEDED-OVER 21.
- Also, if the participant answers the questions on their application as YES to over 18, NO to parents claiming on taxes and NO to parents financially support me they are considered completely financially independent from their parents and they will not need a parental consent form and no email is sent to their parents. You will see it say this on their application: PARENTAL CONSENT FORM NOT NEEDED- FINANCIALLY INDEPENDENT.
