The management of Reliant is committed to providing staff with a safe and healthy workplace. It is the policy of Reliant that staff members report unsafe conditions and do not perform work tasks if the work is considered unsafe. Staff members must report all accidents, injuries, and unsafe conditions to their supervisors or to Human Resourcesthe Reliant Central Office. No such report will result in retaliation, penalty, or other disincentive. Staff members’ recommendations to improve safety and health conditions will be given thorough consideration by our management team. Management will give top priority to and provide the financial resources for the correction of unsafe conditions. Similarly, management will take disciplinary action against a staff member who willfully or repeatedly violates workplace safety rules. This action may include verbal or written reprimands and may ultimately result in termination of employment.

Section IV. ACCIDENT INVESTIGATION

Both incidents (events that caused injury to a person or damage to equipment, building or materials) and near misses (events that could have caused injury to a person or damage to equipment, building, or materials) must be reported to Human Resourcesto the Reliant Central Office.

The supervisor at the location where the accident occurred will work with Human Resources to a member of the Central Office to perform an accident investigation. Human ResourcesThe Central Office is responsible for seeing that the Incident/Near Miss Reports are being filled out completely, and that the recommendations are being addressed. Investigation of all accidents, injuries, and occupational diseases will use the following investigation procedures:

  • The overall area (from all angles) of the accident/incident scene. Start outside and work in. Include the final resting place of the vehicles in relation to the rest of the accident scene. 
  • If an intersection, all four (4) directions approaching that intersection. 
  • Basic four (4) sides of each vehicle. 
  • Close up of damage, or lack thereof, to each vehicle. Photos showing no damage are just as important as photos showing damage. Also include any damage to buildings, signs, etc. 
  • Skid marks, including a point of reference (such as a pen) for measurement. 
  • Any contributing factors (construction, road conditions, etc.) 
  • Any evidence that could be moved later. 
  • Temporary road or construction signs, traffic signals, etc. 
  • Any visible obstructions like trees and shrubs, signs, other vehicles, etc. 
  • License plate number of the other vehiclevehicles involved.

Do your best to photograph any other issues that seem pertinent at the time.
